Make sure you include your order details by filling in, printing and sending this form with your Cheque/Postal Order.
If you do not have access to a printer please include the following information clearly on a piece of paper:
- Your name & full postal address including postcode
- Your email address ¹
- The CD(s) you want and the amount of each
- The total cost (please ensure you add shipping costs)
Please note: If paying by cheque, your order will be posted only after your check has cleared (typically 4-5 working days after the cheque is banked).
